Rexdale Community Health Centre delivers integrated, coordinated clinical services through a team of physicians, nurse practitioners, nurses, dietitians, social workers, a physiotherapist and a chiropodist. Rexdale Community Health Centre is committed to the prevention and management of chronic disease.
Programs and services are based on client education and support with a goal of helping clients achieve healthy lifestyles and improve health outcomes.
Rexdale Community Health Centre provides a range of Health Promotion programs and services which address the social determinants by promoting healthy behaviours; creating supportive, welcoming environments; and encouraging healthy public policies, while enabling people to increase control over and to improve their health.
Programs and services also address health equity issues and remove barriers to access. Our Community Capacity Building programs empower Rexdale residents to achieve their greatest potential and to strengthen the community as a whole. Working individually and in partnership with other agencies, we connect clients to resources that can be helpful to them, advocate for them so that they obtain supports they may need to thrive, assist them to organize and share their perspectives and voice, and help them develop or demonstrate the skills that can support their success.
We are here for Rexdale residents at whatever stage they are in their lives.
